PDO Threads

Polydioxanone (PDO) is a material often used for stitching in surgeries. As a colorless biodegradable synthetic polymer, the threads will be dissolved within a year, but help to stimulate the production of new collagen in your skin which the effects can last until well after the threads have gone.

PDO operates in the cell as mechanical external stimulus, and provides the following effects within the skin:

What is the procedure of this treatment?

Prior to the insertion of the threads, local anesthesia will be given to the treatment area to numb the skin and minimise the discomfort. You will experience a small, pricking sensation similar to acupuncture needling, which is not painful. Depends on the treatment area, procedure can be completed as short as 45 minutes.

How long do the effects last and what will happen to the threads after inserting into the skin?

The threads will be inserted into the subcutaneous layer of the skin. They will be fully absorbed /melted in the skin within 6 months. However, results may last up to 18 months due to the later effects of production of the collagen in response of PDO materials.

The PDO sutures provide immediate lifting effect for sagging skin. As new collagen will be produced gradually, rejuvenating effects such as volume restoration and skin tightening will be more prominent over the next few months. Lines will be softened and better facial contours will be seen.

Is there any downtime after the treatment?

Redness and swelling will resolve in 3 to 4 days. Bruises (if happen) may take approximately a week to resolve. Dimpling and rippling of the skin are commonly seen for a few days after the procedure as the skin is adapting to the newly lifted position.

After the procedure, patients are discouraged from undergoing any dental treatment and performing vigorous exercise for 2 weeks. During this period, it is also advisable to sleep on their back to allow the swelling to subside, and to avoid excessive facial expression and putting too much pressure on the face.

Silhouette Soft?

Silhouette Soft® is produced in the United States by the same company which introduced the Silhouette Lift® sutures in the plastic surgery market in 2006.  Silhouette Lift® threads are suspension sutures which are used in cosmetic and reconstructive surgery.

They have received approval from the US FDA (since 2006) and they are CE marked for the European market.  Since 2007 more than 20.000 Silhouette Lift® procedures were carried out successfully and presently every 15 minutes somebody somewhere in the world is being helped with Silhouette Lift®.

In October 2012 Silhouette Soft® was approved for use in the European market as a “spin-off” of the Silhouette Lift® sutures: they are shorter, with fewer cones and without the need to be stitched to the skin on either end.  They are best described as polylactic acid bi-directional bio-resorbable sutures with cones.

Unlike other existing threads, Silhouette Soft® is made out of a natural component: polylactic acid (PLA), which is derived from renewable resources such as corn starch or sugarcane. Being able to degrade into harmless lactic acid, PLA is used for medical implants in the form of screws, pins and rods (and now also lifting threads). PLA breaks down gradually over a period of 6 to 24 months. This gradual breakdown is desirable for creating a support structure.

How does it work?

Combating the effects of aging means restoring smoother and more toned skin, but also a more shapely face.  To meet this dual requirement Silhouette Soft® combines two effects: lifting and regenerating:

The lifting effect is immediate and discreet as it is the result of compressing and elevating tissue at the time of adjusting the suture.  After inserting the suture, the doctor applies slight pressure on the treated area to reshape the surface of the skin and make it visibly smoother.

The regenerative effect is gradual and natural: polylactic acid (PLA), the principal component of Silhouette Soft® is particularly biocompatible with the human body and completely biodegradable. Once the suture is applied, the PLA acts on the deeper layers of the skin and naturally stimulates the production of collagen. This action which continues over time helps increase the volume of saggy areas and restores shapeliness to the face gradually and perfectly naturally.

Who should go for this treatment?

Silhouette Soft® is recommended for men and women of 30 and over who are trying to combat effectively the signs of aging, which do not want any heavy and invasive treatment, but are looking for real and natural results.

Silhouette Soft® treatment is suitable for all skin types – testing for allergic reactions proved negative in clinical studies.  Nevertheless the use of Silhouette Soft® is not recommended if you are allergic or sensitive to biomaterials.

Who is not suitable for Silhouette Soft?

  • Implants to the face or permanent fillers

  • On steroids

What is the procedure of this treatment?

The insertion of the suture in the subcutaneous (adipose) skin layer occurs through an entry point in the middle of the suture pattern. The entry point enables the insertion of each half of the suture respecting the appropriate orientation of the cones. After the insertion of the sutures in the treated area a manual compression of the adipose tissue is performed to obtain the full and immediate result.

A light local anaesthesia is required only at the entry and the exit points of the sutures.  The sutures are inserted under sterile conditions through the skin in the subcutaneous tissue. The procedure is performed at the office and does not require any surgical incisions or stitches.

How long do the results last?

The results may last average between 12 to 18 months, varied with each individual. A healthy lifestyle and good skincare regime can prolong the effect of the treatment.

Is this treatment safe?

Silhouette Soft is safe because:

  • It received CE 0499 approval and the ISO 1385 standards are in accordance with European Directive 93/42.  Silhouette Lift® is since 2006 already FDA approved in the USA and CE marked.
  • The development of Silhouette Soft® is based on six years of experience in suture suspension with cones used in reconstructive and cosmetic surgery in the United States (Silhouette Lift®).  Silhouette Soft® has been used since 2010 in the Asian market where met great success.
  • Silhouette Soft® is manufactured in Michigan/USA under strict medical and sanitary control.
  • The natural substance of Silhouette Soft® (polylactic acid) has been used for many years in various pharmaceutical and medical applications such as suture thread, orthopaedic pins, screws and nails for bone fractures.  It is also used in new biodegradable stent trials.

What to do before the treatment?

No special preparation is required prior to Silhouette Soft® treatment.

However, we recommend that you discuss your expectations and ask your doctor any questions you have before starting treatment. Inform the doctor of any treatment you have already received and of any medicines you are currently taking.

Is there any downtime after the treatment?

After Silhouette Soft© treatment, as with any cosmetic treatment, some slight swelling, redness or bruising may sometimes be observed, but these disappear within days.

There is sometimes a slight wrinkling of the skin (required for optimisation of the treatment) which disappears very quickly.

Recommendations for good post-treatment care:

  • During the week after treatment, do not undergo any dental surgery, and avoid facial and beauty treatments.
  • Avoid all contact sports for the first few weeks.
  • Preferably sleep on your back.
  • In the event of pain, take a pain reliever according to your doctor’s prescription.

Can I undergo for other cosmetic procedures after the treatment?

Yes, you can. Silhouette Soft constitutes a perfect addition to dermal fillers (Juvederm) and botulinum toxin (Botox). The three different procedures together act as a complete and very efficient armory in the war against aging as they complement the effect of each others and work synergistically.

PDO vs Silhouette Soft - which one is better?

Both PDO and Silhouette Soft threads can provide very good results to improve cheeks and jawline sagging. PDO threads can also be applied on the under eye area to improve eye bags or under eye hollowness, as well as on the nose for a better nose shape.

Both threads have excellent safety profile and similar duration (12 to 18 months) of the results.

Occasionally, these threads can be combined within a treatment to achieve synergistically results.
