Terms & Conditions:
- As a My Bliss Clinic’s referral program customer (a “Referrer”), you agree to our Data Protection Policy & the following terms & conditions. A Referrer must be existing My Bliss Clinic’s customer.
- Qualified Referral
– A Qualified Referral is defined as a transaction made (minimum RM 1000 or above) on his/her first visit at My Bliss Clinic (a “Referred Customer”) who arrives to the clinic by your referral.
– The Referred Customer must inform us the name of the referrer on the Biodata form upon registration.
- The Referred Customer must be first time customer visiting My Bliss Clinic.
- Referral Rewards
– Both Referrer & Referred Customer will be getting e-voucher(s) with RM 100 value per voucher based on the table above. Terms & conditions as stated on the voucher will be strictly applied.
– The voucher(s) can be redeemed only on the subsequent visit within the stipulated validity period on the voucher(s).
- My Bliss Clinic reserves the right to cancel the referral program or to change these terms & conditions at anytime in its sole discretion. Any unclaimed referral rewards will be forfeited at that time.
